These are the headings that various agencies I have been a part of have trained on and have passed insurance company audits (double disclaimer - these are guidelines and are definitely not a guarantee of everything you will personally need in your practice/state/etc.). You or your agency might have slightly different headings but the good stuff - the meat and potatoes - are likely the same!
